STOPS Fast Track, LLC provides vehicle modifications and home enhancements for individuals with disabilities. We have a reputation of excellence for our quality of sales, certified installers and technicians. We work with the insurance industry to meet the needs of injured workers.

In order to properly service this broad group of clients, we are expanding our network of certified contractors to meet this growing demand. We would like to welcome you to apply to be included in our nationwide network. There is no fee to apply or to be a member of the Fast Track team.

As a vendor for Fast Track, we require that you review and sign the Contractor Agreement, complete the Contractor Profile, and supply us with copies of the following:

• Licenses including trade, local, business, state and county licenses, and any certifications provided by member service professionals as applicable.
• Completed W9 Form
• Certificate of Insurance indicating at least $1,000,000 of liability coverage and evidence of Worker's Compensation with STOPS Fast Track, LLC listed as certificate holder.
• If you have any special certifications please include them as well and list at least two references.
• If you have a company website, please provide us that information and include a brochure if you have one.
